Forms and Applications

The City of Edwardsville has a number of forms and applications for residents and visitors to complete for various purposes. Below you will find a listing of some of these forms. In order for your form or application to be processed, please print these forms off and return to City staff at Edwardsville City Hall at 409 S. 4th St., Edwardsville, KS 66111. Utility Bill Forms

  • Utility Billing Fact Sheet: Reference this form for the most up-to-date information regarding utility billing in the City. This includes due dates, services offered, delinquent account policies, and more.
  • Utility Billing Residential contract: If you are a new resident or need to update account information, please fill out this form for staff to keep on file. This will serve as the formal authorization for City utility service (trash/recycling pickup and sewer if applicable) to begin.
  • Utility Billing auto-payment form: At this time, the City can only accept auto payments through a customer's banking information. To register a new account with auto-payment or to change you existing account to autopayment, please complete this form and return to the Customer Service Clerk at City Hall. Signing up for auto-payment also comes with a one-time $10 savings on your next utility bill.
  • Utility Billing Commercial contract: If you are a new or existing Edwardsville business and would like to set up sewer service, please fill this form out for the formal authorization to begin service.
  • Multi-Family/Landlord Utility Billing Contract: If you are a property owner with multiple domiciles in the City, and would like to begin City utility service, please complete these form and return to City staff.

Park Rental Forms

  • Community Center Rental form: Use this application to request a rental date for the Edwardsville Community Center. Submission of this form does not guarantee a rental date. Dates are not secured until the required deposit is paid and rentals are not finalized until all fees are paid. Rentals with unpaid fees are subject to cancellation.
  • Park Shelter Rental form: Use this application to request a rental date for the Edwardsville City Park Shelter (1200 Blake St.). Submission of this form does not guarantee a rental date. Dates are not secured until the required deposit is paid and rentals are not finalized until all fees are paid. Rentals with unpaid fees are subject to cancellation.
